Nutrition Pitfalls in the Transition of Care for Chronic Kidney Disease (CKD) and Acute Kidney Injury (AKI) is organized by National Kidney Foundation (NKF).
Overview:
In this activity, faculty will address the topic of malnutrition in CKD and AKI, with a focus on transitions of care. Issues regarding the causes and effects of malnutrition in these populations will be discussed, along with evidence-based practice for remediating inadequate nutrition care.

Learning Objectives:
Explain how malnutrition affects outcomes in patients with chronic kidney disease (CKD) and acute kidney injury (AKI)
Discuss the origins of inadequate nutrition that contribute to protein energy wasting (PEW) in the CKD and AKI populations, including issues related to transitions of care
Describe the special nutrition and fluid requirements of the patient with AKI in the outpatient dialysis setting
