Chronic Kidney Disease is of high prevalence, increases with age, and is a great burden at end-stage. Patient outcomes are known to be significantly improved with early identification and intervention, but CKD remains underdiagnosed and undertreated. There are numerous barriers to early identification, including patient failure to engage, inadequate and conflicting professional guidelines, and the failure of payers to cover the costs of screening and early intervention. This monograph aims to assist PCPs to overcome the barriers and promote early identification, timely referral, and successful comanagement of the CKD patient.
Learning Outcomes/Objectives:
Upon completion, participants should be able to:
• Describe the harms of late identification and late referral of patients with Chronic Kidney Disease (CKD).
• Identify the barriers to early identification and intervention in patients with CKD.
• Construct and implement an evidence-based policy for your practice to screen for CKD.
• Evaluate patients and prioritize treatment strategies to meet appropriate goals of CKD management.
• Construct and implement an evidence-based policy for when you will refer your CKD patients to a nephrologist.
