Course Description:
Thousands of patients in countries worldwide are awaiting a new kidney; more than 100,000 in the U.S. Kidney transplantation provides an opportunity to leave the rigors of dialysis behind and improve the quality of life for chronic kidney disease patients. The purpose of this activity is to address the importance of collaboration between dialysis centers and transplant programs resulting in awareness of the benefits of kidney transplantation and early referral.
Learning Objectives:
• Describe CMS CoP for annual reporting/communication between dialysis units and transplant centers
• Improve dialysis providers knowledge for delivering transplant education and discuss selection criteria for transplantation
• Discuss ways to strengthen relationships between transplant and dialysis centers to facilitate transplantation
