Description :
Increased reporting of medication safety events by all practitioners within our health system. Lack of knowledge related to the impact that reporting all medication events, regardless of level of harm that may occur to the patient, can have on system-level safety initiatives and institutional safety culture.
Objectives :
Upon completition of this activity, participants will be able to:
• Describe the evolution of medication event reporting
• Discuss terminology related to medication events
• Outline barriers to medication event reporting
• Explain the importance of reporting medication events by all staff members
