• Recommend a therapeutic transition between warfarin and a direct oral anticoagulant (DOAC).
• Recommend a treatment regimen for venous thromboembolism (VTE) in obesity based on the International Society on Thrombosis and Haemostasis 2021 guidelines.
• Apply current guidelines on prophylaxis and treatment of VTE in patients with cancer to a patient case.
• Recommend an appropriate adjustment to a warfarin regimen in a patient with an out-of-range INR.
