Medication errors are an important issue facing the healthcare system. Experts have increasingly recognized that errors can be prevented by making the medication use system safer rather than relying on individual healthcare providers to always perform perfectly. Educating pharmacy teams about the science of medication safety is an important first step in helping teams develop a systems view of their care settings and a mindset that embraces continuous learning.
Objectives:
Upon successful completion of this knowledge-based course, pharmacists should be able to:
• Identify product design features that predispose end users to medication errors
• List ways to minimize or eliminate product design hazards from the practice environment
• Define latent failure and describe how latent failures lead to medication use errors
• Apply proactive risk assessments in pharmacy practice
