The roles of temporal artery biopsy, ultrasound, and imaging for large vessel vasculitis (LVV) are illustrated and discussed. Initial therapy with intravenous or oral high-dose corticosteroids is reviewed with consideration for vision-threatening cranial GCA. Evidence-based practical use of steroid-sparing agents, specifically tocilizumab, is discussed in the context of current American College of Rheumatology/Vasculitis Foundation treatment guidelines for GCA.
