Their continuum of care services integrate social and medical aspects of care to achieve the best outcomes at the lowest costs for healthcare partners. During this webinar participants will learn where to acquire adult protective and healthcare services using the Illinois Department on Aging (IDoA) site, community services and supports that address the social determinants of health.
