Overview:
Root canal treatment is a highly successful procedure. However, the treatment of teeth with immature roots and pulp necrosis presents significant difficulties for the dental clinician in regard to disinfection and filling of the wide root canal space and open apex. Traditional approaches involved apexification by long-term medication with calcium hydroxide to create a calcified barrier at the level of the open apex, or a short-term apexification with artificial materials as an apical stop, each providing for a predictable root filling after the procedure was completed. However, these traditional techniques often resulted in subsequent root fractures due to the thin root walls remaining even after successful apexification procedures.
Today, for the practicing clinician, the revascularization of immature necrotic teeth has become an alternative to apexification. Current techniques involve disinfecting irrigation, placement of calcium hydroxide or an antibiotic paste as a temporary medication, and the instigation of a blood clot within the root canal space to establish the revascularization process with continued root growth, closure of the apex, and thickening of the root walls. The lecture discusses advantages and disadvantages of traditional apexification versus current revascularization techniques and reviews the correct procedure step-by-step both on a biological and clinical basis.
