Goal:
To provide pharmacists with skills and knowledge needed to develop a Personal Medication List (PML) and Therapeutic Action Plan for patients undergoing medication therapy management (MTM).
Educational Objectives:
Upon completion of this activity, participants should be better able to:
• Identify components and goals of a personalized Medication List and To Do List (TherapeuticActionPlan)
• Review recommended forms or templates for specific patient needs and circumstances
• Review best practices for documenting and sharing the Medication List, To Do List, and other MTM documents with other stakeholders including healthcare providers
