Goal:
To provide pharmacists and pharmacy technicians with practical approaches for improving medication safety through medication error prevention. Guidance will be provided for those in community and health-system settings. Additionally, attention to special populations including pediatric and geriatric patients will be given.
Educational Objectives:
Upon completion of this program, participants should be better able to:
- DESCRIBE the rate of occurrence of medication errors among older adults and the related health care and societal burdens.
- LIST factors that put older adults at risk for experiencing adverse drug events.
- DEFINE polypharmacy and medication overload and contributing factors.
- EXPLAIN the purpose of the Medication Appropriateness Index, AGS Beers Criteria, and STOP/START Criteria.
- DESCRIBE three medication interventions and/or strategies to optimize medication use in older adults.
