However, institutional medication history training for pharmacy technicians can be inconsistent and scarcely available within health systems, creating a barrier for pharmacy technicians to advance their practice. Literature has shown that pharmacy-technician obtained medication histories lead to better high-risk discrepancy identification and fewer medication reconciliation-related errors. Currently, this process is not led by the pharmacy team at every institution. It is our goal to provide a medication reconciliation tool and CE presentation to help pharmacy technicians advance their roles as medication reconciliation techs.,,Objectives:,Upon completion of this activity participants will be able to:,• Discuss the need and goals for advanced pharmacy technician medication reconciliation roles;,• Review the medication reconciliation process and how to conduct a patient interview;,• Explore the concept of using a medication reconciliation card deck tool to help prepare novice pharmacy technicians to find problem prone discrepancies in a medication history that might lead to medication errors or patient harm.
