Inadequate or inaccurate medical record documentation raises the risk of medical errors and legal liability. Reliable and timely medical record documentation helps diagnosis and treatment, communicates pertinent information to other caregivers to ensure patient safety, and supports optimal care delivery and patient outcomes.
