Key guideline highlights:
• Asymptomatic postmenopausal women and men with iron deficiency anemia should undergo bidirectional endoscopy in an attempt to identify a bleeding lesion; esophagogastroduodenoscopy and/or colonoscopy (or vice versa) should be performed at the same setting.
• Consider bidirectional endoscopy in asymptomatic premenopausal women with iron deficiency anemia without other plausible etiologies. Because the risks and benefits of bidirectional endoscopy vary by patient age, shared decision making is needed.
• Non-invasive testing for Helicobacter pylori, followed by treatment if positive, is suggested.
• Initial serologic testing for celiac disease, with small bowel biopsies only if positive, is suggested over performance of routine small bowel biopsy during esophagogastroduodenoscopy.
• In uncomplicated asymptomatic patients with iron deficiency anemia and no source of potential blood loss or iron malabsorption identified after initial evaluation and bidirectional endoscopy, a trial of iron supplementation is recommended before routine evaluation of the small bowel with video capsule endoscopy.
